Mark your calendars now for Fall Sports Paperwork
Nurses will be at Southtowns Catholic School to accept sports paperwork on the following dates;
Tuesday, August 15 8am - 10am
Wednesday, August 16 6pm - 7:30pm
Wednesday, August 23 10am - 12 Noon
Please bring in updated physical forms.
The nurse will have a Code of Ethics, a Parent Consent & Health office update questionnaire form and a Sports Emergency Information / Parental Permission form.
Every athlete needs a parent permission form and a current physical to be eligible for sports. The athletic director will notify the coach when complete.
DO NOT EXPECT THE COACH TO MAKE ANY EXCEPTIONS. No one can play without completing the paperwork.
